Acknowledgment of Risk

I, the undersigned, understand and acknowledge that participation in tennis activities, lessons, or events at [Hill Tennis Academy] involves inherent risks, including but not limited to:

Physical injury (e.g., sprains, fractures, or other injuries).

Risks associated with the use of equipment or facilities.

Risks arising from the actions of other participants or spectators.

I voluntarily choose to participate (or allow my child/ward to participate) in activities at [Hill Tennis Academy] with full knowledge and understanding of these risks.

Release of Liability

In consideration of being permitted to participate in activities at [Hill Tennis Academy ], I hereby release, waive, discharge, and hold harmless [Hill Tennis Academy], its owners, employees, coaches, contractors, volunteers, and affiliates from any and all liability, claims, demands, actions, or causes of action arising out of or related to any loss, damage, or injury, including death, that may be sustained by me (or my child/ward), whether caused by negligence or otherwise.

Medical Consent

I authorize [Hill Tennis Academy ] and its staff to provide or arrange for emergency medical treatment in the event of injury or illness during participation. I understand I am responsible for any medical expenses incurred.

Photography/Media Release 

I grant permission for [Hill Tennis Academy ] to use photographs or videos of me (or my child/ward) for promotional purposes, including social media, website, or other marketing materials.

Acknowledgment of Understanding

I have read and fully understand the terms of this liability waiver. I understand that by signing this document, I waive certain legal rights, including the right to sue, and voluntarily agree to its terms.
